    Peer Support

At  Hope ’n Wellness , Peer Services are delivered by Certified Peer Counselors who use lived experience to offer non-clinical, side-by-side support. Together we can clarify goals, build skills, navigate systems, and celebrate wins, however small. Expect trauma-informed care, cultural humility, and zero-barrier access. Services can include weekly check-ins, wellness planning, appointment accompaniment and advocacy, and connections to housing, benefits, and community resources. Wherever you’re starting, our peers meet you there and walk with you toward where you want to be.

If you are an agency, looking to refer someone for Peer Services, you can find our referral form at the link below.

    Foundational Community Supports

Foundational Community Supports (FCS) is a program designed to help people with complex health and social needs access supportive housing and employment services through Apple Health.

At Hope ‘N Wellness, we partner with  Wellpoint  to provide compassionate, consistent case management that focuses on more than just checkboxes. We’re here to help you create a stable foundation, whether that means finding safe housing, preparing for employment, navigating systems, or simply having someone walk beside you who cares.

FCS is about more than services, it’s about support, trust, and believing in what’s possible. We work together to set goals, overcome barriers, and connect you to the resources you need to move forward with confidence.

FCS services are available by appointment, require  Wellpoint  approval, and ongoing appointments with a case manager.
